HOW TO TAKE THE OSTRC H2 QUESTIONNAIRE FOR ATHLETES

ATIUM Sports Use The OSTRC H2 Questionnaire Every Monday. The OSTRC Questionnaire is an essential tool for improving injury surveillance and health monitoring in sports. It offers valuable insights into athlete well-being, enabling proactive health management. By integrating this questionnaire, teams and health professionals can more effectively track and support athletes, enhancing their overall performance and health.


Step 1: Open the ATIUM Tasks App

  • Launch the "ATIUM Tasks" mobile app on your device.

Step 2: Home Screen Notification

  • Every Monday, you will see a notification on your home screen prompting you to complete the OSTRC H2 Questionnaire.

Step 3: Access the Questionnaire

  • Tap the "OSTRC Questionnaire" button on the home screen.

Step 4: Fill Out the Questionnaire


Q1. Have you had any difficulties participating in training and competition due to injury, illness, or other health problems during the past 7 days?

  • Full Participation without health problems: You trained and competed without any health issues.

  • Full Participation but with a health problem: You trained and competed despite of having health  health issues (e.g., a minor cold).

  • Reduced Participation due to a health problem: You trained and competed less because of a health issue (e.g., mild sprain).

  • Could not participate due to a health problem: You train or compete at all because of a health issue (e.g., a significant injury).

Q2. To what extent have you modified your training or competition due to injury or other health problems during the past 7 days?

  • No modification: You didn't change your routine of traning and competition at all.

  • To a minor extent: You made slight changes (e.g., reduced training intensity).

  • To a moderate extent: You made moderate changes (e.g., skipped some training sessions).

  • To a major extent: You made significant changes (e.g., avoided competition entirely).

Q3. To what extent has injury, illness, or other health problems affected your performance during the past 7 days?

  • No effect: Your performance wasn't affected.

  • To a minor extent: Your performance was slightly affected (e.g., felt a bit tired).

  • To a moderate extent: Your performance was moderately affected (e.g., slower than usual).

  • To a major extent: Your performance was greatly affected (e.g., couldn't perform well at all).

Q4. To what extent have you experienced symptoms/health complaints during the past 7 days?

  • No effect: No symptoms at all.

  • No symptoms/health complaints: Felt completely fine.

  • To a minor extent: Minor symptoms (e.g., mild headache).

  • To a moderate extent: Moderate symptoms (e.g., mild fever).

  • To a severe extent: Severe symptoms (e.g., significant pain or illness).

After answering the questionnaire, you'll be directed to a page where you need to provide specific details about any injuries you may have. 

On the next you have to enter the Type of Problem


Acute Injury

Sudden injuries caused by a specific incident or immediate impact.

  • Examples:

    • Hamstring Strain: A sudden pull or tear in the back of your thigh, often occurring during activities like sprinting or jumping.

    • Ankle Sprain: Twisting or rolling your ankle, causing immediate pain, swelling, and difficulty walking.

Overuse Injury

Injuries that develop over time due to repetitive movements or stress on a specific part of the body.

  • Examples:

    • Tendonitis: Inflammation of a tendon, commonly occurring in athletes who perform the same motion repeatedly, such as runners or swimmers.

    • Stress Fractures: Tiny cracks in bones caused by repeated impact or overuse, often seen in long-distance runners or dancers.

Other Injury

Injuries that do not fit into the categories of acute or overuse, often related to chronic conditions or non-sport-related issues.

  • Examples:

    • Chronic Back Pain: Persistent pain in the lower back that can be caused by muscle strain, disc problems, or other spinal issues.

Illness

Health problems affecting various body systems beyond just bones and muscles, including issues related to respiratory, digestive, neurological, psychological, or social health.

  • Examples:

    • Respiratory Issues: Conditions like asthma that impact breathing and respiratory function.

    • Digestive Problems: Issues such as irritable bowel syndrome (IBS) affecting digestion and gastrointestinal health.

    • Neurological Conditions: Disorders like migraines or epilepsy that affect the nervous system.

    • Psychological Disorders: Mental health issues such as depression, anxiety, or stress-related conditions.

    • Social Problems: Issues affecting social well-being, like difficulties in personal relationships or social isolation.

After you have detailed your injury, you will be directed to another page

Here You Have to mention that,  Did you Already Report the Problem?


  • Yes: Select this if you have already informed your coach, medical team, or relevant authority about the injury.

  • No: Select this if you have not yet reported the injury to anyone.

(Note: If the injury has been reported on the ATIUM app by the athlete or the medical team, selecting "Yes" will display the reported injuries for the athlete to choose from.)

Area of Injury

  • Head: Select this if your injury is located on your head.

  • Wrist: Choose this if your injury is on your wrist.

  • Hand: Choose this if your injury is on your hand.

  • Knee: Choose this if your injury is on your knee.

  • Elbow: Choose this if your injury is on your elbow.

  • Ankle: Choose this if your injury is on your ankle.

  • Shoulder: Choose this if your injury is on your shoulder.

  • Other: Select this if your injury is on another part of your body and specify the exact location.

  1. Mention Left/Right Side in Case of a Localized Injury

  • Left: Select this if the injury is on the left side of your body (e.g., left ankle).

  • Right: Select this if the injury is on the right side of your body (e.g., right knee).

  • Not Applicable: Select this if the injury is not specific to one side of your body (e.g., back pain).

  1. Any Additional Comments for This Injury?

  • Enter Comments...: Use this space to provide any extra information about your injury. For example:

    • How the injury happened (e.g., "Fell during training")

    • How it feels (e.g., "Sharp pain when moving")

    • Any treatment or care you are receiving (e.g., "Using ice packs and resting")

  1. Finish

    • Tap this button once you have entered all necessary details about your injury. This submits your injury report for review by your coaches or medical team.


After you have detailed your injury, you will be directed to another page,There You Have to Indicate the Number of Days of Complete Loss (Total Inability to Train or Compete) Due to the Reported Problem

  • Number of Days...: Enter the total number of days you were completely unable to train or compete due to the injury. For example:

  • If you could not train or compete for 3 days, enter "3".

  • If the problem didn’t cause you to miss any training or competition, enter "0".

  • Tap the “FINISH”button once you have entered all the required details about your injury and the number of days lost. This finalizes your injury report for submission.


Step 5: Submit Your Responses

  • Once you have completed all the questions, tap the "Submit" button.

Answer all the questions honestly and thoroughly. The questionnaire covers your health status, any pain or injuries, and how these affect your training and performance. Once reported, the data will be made available to the medical team and coaches to make informed decisions regarding the health of the team.
